CAMPS REGISTRATION

Call to reserve space before attending any camps.

To Register: Submit registration form via Email , Mail or Fax.
Important: Registration form is only VALID once deposit is received.


 

CAMP RESERVATION POLICIES:

Payment: Payment in full is required for all camps.
Call for early enrollment deposit arrangements (anything prior to 2 months before camp start date): 310-314-0035
If payment is late then we reserve the right to give other campers on the waiting list the spot.

       
Enrollment: Early enrollment is encouraged as camp spaces are limited.
A player may join camp after it has started if there is space available.
Please call before to check on space availability and fees.

     
Refunds:                         Should you withdraw from camp prior to 10 days before the camp start day, you will receive a full refund minus the non-refundable and non-transferable $135. After that date, there will be no refunds.

   
Prorates and Transfers: There are no refunds or prorating for campers time off, family vacations, special events in your child's life, or anything else that necessitates your child's missing camp once s/he has been enrolled.

   
Cancellations: KOS reserves the right to cancel a camp if enrollment is insufficient (early enrollment helps assure class availability).
If paid, KOS will refund in full or can transfer to any class or camp.

In consideration with the applicant's participation in the above activity, I waive and release all claims for damages, death, person injury, or property damage that may occur as a result of engaging in that activity. This discharges in advance Kids On Stage, Inc., it's employees and other agents for liability. I know that this activity involves a risk of accident and I willingly assume the risk. This waiver, release and assumption of risk are binding on my heirs and assigns. I hereby release and hold harmless Kids On Stage, Inc, it's instructors and employees for any claims. I give permission for any medical care that the leaders of the above deem necessary. I hereby consent to the photographing, recording or reproduction of any other manner of the likeness, voice and or activities of my child and further authorize Kids On Stage, Inc. to make unlimited use of reproductions. I understand I will not receive any monetary compensation, now or in the future of participating. I have read and understand all the policies:
