REGISTRATION AND PAYMENT:

To Register: Submit registration form via Email , Mail or Fax.
Important: Registration form is only VALID once deposit is received.


 

CLASS RESERVATION POLICIES:

Payment: Payment in full is required BEFORE the first day of class.
A $130 deposit for early enrollment will hold class space until payment is due in full.
New students may pay for one trail class but spaces are only guaranteed with full session payment.
Students may not attend class without payment.
Payment by credit card is not available at this time.

             
Register: Online or via fax or mail (see below).
Registration form is only VALID once deposit is received.
There is a $25.00 charge for late payments or returned checks.

     
Enrollment: Early enrollment is encouraged as class spaces are limited.
Students are required to enroll for the entire session.
However, a student may join class after a session has begun if there is availability.

   
Winter/Spring Winter and Spring Session are two separate session payments although they are connected.
Sessions Info:Money will be due for Winter and then for Spring.
Winter/Spring performances are in May/June.

   
Refunds: Refunds will be given only if a student cancels BEFORE the session begins minus the $100 deposit and a $30 registration fee once actor has begun.
Refunds will not be provided for students who miss class for any reason (i.e. sick, travel, conflicts etc.).
Only in certain cases, class credit towards classes and camps may be allowed for one year from enrollment date. Credits not valid for privates, custom, workshops, parties, or any class or camp per the Program Director's discretion.

     
Cancellations: KOS reserves the right to cancel a class if enrollment is insufficient
(early enrollment helps assure class availability).
Refunds will be prorated if necessary.

Waiver:

In consideration with the applicant's participation in the above activity, I waive and release all claims for damages, death, person injury, or property damage that may occur as a result of engaging in that activity. This discharges in advance Kids On Stage, Inc., it's employees and other agents for liability. I know that this activity involves a risk of accident and I willingly assume the risk. This waiver, release and assumption of risk are binding on my heirs and assigns. I hereby release and hold harmless Kids On Stage, Inc, it's instructors and employees for any claims. I give permission for any medical care that the leaders of the above deem necessary. I hereby consent to the photographing, recording or reproduction of any other manner of the likeness, voice and or activities of my child and further authorize Kids On Stage, Inc. to make unlimited use of reproductions.
